It may happen that you do not get the gem. It is not clear why, but the following things have almost always helped.
- First click the quest then go to the general store
- Keep only 98 livers of tigers in your inventory and get the two new livers
- Wait a few days
Any one of those could clear a bugged quest. If you do not have the sewing thread and the jewel the quest cannot be completed
